Insulin sensitivity can have a big impact on body composition. When your body responds well to insulin, it can regulate blood sugar and use nutrients more efficiently. But it’s only one piece of the puzzle—nutrition, activity, sleep, hormones, genetics and overall energy balance all matter too.

It’s a real concept, but I wouldn’t think of it as a permanent “safe” form of obesity. Some people with obesity can have relatively healthy blood sugar, blood pressure and cholesterol for a period of time, but their risk isn’t necessarily the same as someone without obesity. Metabolic health can change over time, which is why it’s more useful to look at the whole picture—blood markers, blood pressure, fitness, lifestyle and waist circumference—rather than weight alone.

One of the biggest mistakes is treating GLP-1 medication as a quick fix rather than one part of a bigger health plan. Honestly, it’s not really an either-or. All three matter, and they work together. Muscle helps you stay strong and independent as you age, cardiovascular fitness is strongly linked with long-term health, and good metabolic health reduces the risk of many chronic diseases. If I had to pick a priority, I’d focus on building a good level of cardiovascular fitness and maintaining muscle while keeping your metabolic health in check. The best longevity strategy is usually a combination of all three.

Psychological stress can have a bigger impact on metabolic health than people realize. Chronic stress can disrupt sleep, increase cortisol, affect appetite and cravings, and make it harder to maintain healthy habits. Over time, these changes can contribute to insulin resistance, weight gain and other metabolic problems. It’s not about eliminating stress completely - it’s about having good ways to manage it and giving your body enough time to recover.
